Learn more about our Youth in Transition Workers program below!
The Youth in Transition Worker (YITW) Program provides one-on-one support to youth who are transitioning out of the child welfare system. The program helps youth navigate adult services, build independence, and access the resources they need to successfully transition to adulthood.
Individualized transition planning
Support with housing, education, employment, and life skills
Referrals to mental health and addictions services
Assistance accessing income supports and health services
Advocacy and system navigation
Culturally responsive and trauma-informed support
Emotional support and mentorship
Youth aged 16–24 years who are:
Current or former youth in care (Crown wards, Ready Set Go -RSG, Voluntary Youth Service Agreements – VYSA)
Living independently or preparing to exit care
Self-referral by youth
Referrals from CAS, VYSA Workers, RSG Case Managers
Support youth in developing the skills, and connections needed for independence
Improve access to stable housing, education, and employment
Foster self-advocacy and goal setting
Strengthen social, cultural, and community connections
Reduce homelessness, crisis involvement, and service disconnection for youth leaving care
